Intake Hours: 9:00am - 1:00 pm (doors will close at 1pm sharp!)

You will need to bring in the following informatiion:

Photo ID - verifies who you are
Mass Health Card - verifies all members in your family
Utility Bill - Verifies where you live (Phone, gas, heat, electric, cable, etc.)
Birth Certificate for each child under 15 years of age - verifies the age of the child

If you do not have a Utility Bill or Mass Health Card, you must bring:
Lease Agreement - verifies all members in your family and verifies where you live

OR

Free & Reduce mean notification from DTA or letter from each child's school, with address verifies that the child lives with you
